Eau Claire County Screen Liaison
Eau Claire County Screen Liaison : Responsible for supervision of Aging and Disability Resource Center (ADRC) staff providing information and assistance, long-term care options counseling, and advocacy to individuals age sixty and older, adults with physical and developmental disabilities, adults with mental health and substance use disorders, young adults with disabilities who are preparing to transition into the adult service network, and their families and caregivers. Projected starting rate of $21.81 to $24.01 per hour plus excellent fringe benefit package. Bachelor’s degree with a major in social work, sociology, human services-related field, or other closely related field or a Registered Nurse in Wisconsin, pursuant to Wis. Stats. §441.06. 1 yr paid full-time experience working directly with people age sixty and older, individuals physically or developmentally disabled, or individuals with mental health or substance use disorders. Possess the DHFS certification for administering functional screens with 1 yr experience administering the Department of Health and Family Services (DHFS) long-term care functional screen. 2 yrs supervisory experience.
